The NC DWQ Stream Identification Form Version 4.11 is used to classify streams as ephemeral, intermittent, or perennial based on a point system evaluating geomorphology, hydrology, and biology.
This form aids in environmental regulation, development planning, and stream health assessment, ensuring accurate stream classification for regulatory compliance and conservation efforts.

1. Each section has select wheels to indicate if something is absent, weak, moderate, or strong.
2. A score will automatically be given to each section to then determine if a stream is ephemeral, intermittent, or perennial.
Key Sections
1. Geomorphology: This section evaluates physical characteristics like the channel structure, sediment type, and floodplain features. The goal is to understand the stability and physical form of the stream.
2. Hydrology: It assesses factors like baseflow presence, soil evidence of high water, and sediment on vegetation. These indicators help determine how often water flows through the stream.
3. Biology: Biological evidence such as aquatic life, plant roots, and algae is considered. More diverse and abundant biological activity usually indicates a perennial stream.
Stream Classification
- Ephemeral: Minimal flow, typically dry most of the year.
- Intermittent: Seasonal flow with scores between 19-29.
- Perennial: Year-round flow with scores of 30 or more.
